New Patient Paperwork

Please arrive at least fifteen minutes early to complete your new patient paperwork. Our forms are also available online so you can print and fill them out ahead of time if you prefer.

Remember to list current medications and known health conditions, as this information is vital to providing personalized care.

We also encourage you to bring a list of any questions or concerns.

Insurance Information

Bring your insurance card and ID with you. We accept many major dental insurance plans

Check-In and Review

We create a friendly, professional atmosphere for our patients. If you have any anxiety about seeing the dentist, please let us know. We’re here to help put you at ease. We’ll review your history and go over your concerns and goals.

Digital X-Rays

Digital X-rays are an important tool to detect hidden issues in your mouth such as tooth decay or infection. A hygienist will take images of your mouth using this advanced technology.

Professional Cleaning

Your hygienist will do a thorough cleaning to remove plaque and tartar buildup, then they will polish and floss your teeth. Unless otherwise directed, brush and floss normally before and after your appointment. If you have questions about oral care techniques, we’ll be happy to show you the most effective ways to clean your teeth at home.

Comprehensive Exam and Treatment Plan

The dentist will review your X-rays and examine your teeth, gums, jaw and bite to develop a personalized care plan for your oral health. The dentist will go over their findings, answer your questions and discuss next steps.
